When completing the forms, please keep in mind the following information:

• Please have an authorized officer of your company sign, notarize, and return the original document to our offices, so that we can release your check.
• Fill out the entire form completely (including amounts and dates).
• Incomplete documents and faxed copies are not accepted.
• Please mail your release to our Main Office to the attention of Accounts Payable at the following address: 12231 Parklawn Drive, Rockville, MD 20852-1723
• Do not come to our main office to pick up checks, as they will not be released.
